Shiver Kindergarten Registration

Registration for Shiver Kindergarten 4/24 & 25/2012
Shiver's kindergarten registration for the 2012-2013 school year will be held April 24th and 25th, 2012. Please call the school now to schedule an appointment time (229-377-2325). Registration on the 24th will be from 8:30 a.m. until 2:30 p.m. and on the 25th from 12:30 p.m. until 2:30 p.m.
You are asked to bring the following:
- Original/Certified Birth Certificate or other proof of birth (hospital record of live birth)
- Social Security Card
- Georgia From 3300 (Vision, Hearing and Dental Screening must be signed by the physician or a registered nurse)- or, if your child is under 4 years of age, you will need to bring an appointment card for the screening from your child's physician or Health Department
- Georgia Form 3231 (Immunization)
- Proof of Residency (example: copy of current bill/current lease/insurance bill addressed to you) (We cannot accept phone/cell bill or driver's license)
Keep in mind the student must attend registration to complete a quick screening.
